Call your insurance company to verify that the Northwest Women’s Clinic provider you wish to see is contracted with your insurance plan. You may give them our tax id number # 93-0612706, and they can tell you if we are in network as a preferred provider. (If needed, obtain a referral from your primary care physician.)
Step 2: Check Hospital Coverage
If you are pregnant, make sure that either Providence St. Vincent’s Hospital or Legacy Good Samaritan Hospital are covered under your plan. These are the two hospitals where we deliver babies and perform surgery.

If this is your first visit, or the first time you have seen us in the last three years, please fill out the forms sent to you through a link either by email or text two days before your appointment
On the day of your appointment arrive 15 minutes early and bring: · Your insurance card and photo identification. · A list of questions or concerns you may have. · A list of medications you are currently taking, · A form of payment for services your insurance may not cover. We accept cash, checks, and credit cards (Visa, Mastercard, American Express, and Discover).
